There are two options for submitting conference registration forms.
Option 1: Fillable Microsoft WORD form -- click below
Can be saved and submitted electronically, or printed and
submitted by mail.
Option 2: Fillable PDF form -- click below
Can be saved and submitted electronically with Adobe Acrobat or
Adobe Acrobat Pro (not
Adobe Reader), or printed and submitted by mail.
Refund Policy: A total refund of conference fees will be given if notification is provided in writing one month before the conference begins. After that time, no refunds will be given.
